Good evening PVA Parents and Guardians,
It has come to our attention that there is some confusion regarding the returning student registration forms. Please note that there are two documents that need to be completed.
1. The PVA Registration Form and Palm Valley Academy Placement Card is an internal document for the use of the Palm Valley Academy Administration Team. This was a printed paper that was sent home via your child’s backpack and was to be returned to school.
2. The School District Returning Student Verification Form is a St. Johns County District form that needs to be completed electronically. Every family received instructions to complete this form via email over the last few days.
We do apologize for any miscommunication that the double forms have caused and will make every effort to streamline this process for next year.
Both the paper form you received from Palm Valley and the electronic form you received from the School District are essential documents that need to be completed for each student in your household.
