If you need to add anything to your student\u2019s health record, please send in a <em>written note (\u201cAttention: School Nurse\u201d)<\/em> to be placed with your student\u2019s file.<\/li>\n<li><strong>Need to leave medication at school?<\/strong> Download the appropriate forms (right hand column) and bring into your pediatrician. A parent <em>and<\/em> doctor must sign the forms for them to be valid. Only <strong>new, unexpired<\/strong> medications can be accepted. Regulations require all medications to be in their original container with a readable pharmacy\/consumer label. Please see our Medication Policy (right hand column) for any further questions.<\/li>\n<li><strong><a
